Whether you are coming in for your first appointment or returning for care, we want the process to feel clear and straightforward. You are welcome to call the office with any questions before your visit.
Bring your insurance card
Bring your referral/prescription if available
Bring imaging reports if available
Bring completed forms if you’ve filled them out in advance
Wear comfortable clothing that allows easy movement
Insurance & Payment
Payment is expected at the time of your visit. Payment by cash or check is appreciated; a 3% surcharge is placed on payments by credit card.
We accept a wide range of insurance plans.
We participate with most major and regional insurance carriers. Because plans may vary, we recommend calling the office with your insurance information prior to your appointment so we may help verify participation and co-pay requirements.

In New York State, many patients can begin physical therapy without first seeing a physician for a prescription.
This means you may be able to come directly to North Fork Physical Therapy for an evaluation and treatment.
Direct access may apply to acute or chronic musculoskeletal injuries involving the shoulder, elbow, wrist, hand, neck, back, hip, knee, ankle, or foot. Following your evaluation, your physical therapist will determine the best plan of care and let you know if additional medical testing or referral is needed. For your safety, we will always refer you to the appropriate physician if we feel a more serious condition may be present.
