Choosing A Physical Therapist

At North Fork Physical Therapy, PC you are the most important member of your our health care team, and we encourage you to choose the most appropriate health care professional to meet your goals.

Freedom of Choice

You have the freedom to choose your own physical therapist. New York State allows you to go directly to a physical therapist without getting a physician's referral first. Keep in mind that your insurance policy may require a visit to the primary care physician first or may limit your access to preferred providers only. Some health care providers refer patients to physical therapy practices that they have a financial interest in.

You have the right to choose your own physical therapist,and you are not obligated to receive physical therapy in any specific facility.

The clinicians at North Fork Physical Therapy, PC are members of APTA and Board Certified Clinical Specialists in Orthopaedics. As such, they are bound by the Association's Code of Ethics and are especially committed to providing competent and compassionate care.

What You Can Expect from North Fork Physical Therapy, PC

At North Fork Physical Therapy, PC you will receive a physical therapy evaluation from a licensed physical therapist, who will thoroughly explain your condition and discuss your treatment options with you. Your participation is vital to the process, and you should feel comfortable asking your physical therapist any questions regarding your plan of care, interventions, and expectations.

Our staff will notify you regarding whether we participate with your insurance company and whether or not you are responsible for a copay or deductible. We are participating providers for most insurance companies, which can often minimize your financial responsibility.
